Our whistle-blowing form provides a secure platform to encourage staff and relevant stakeholders to report suspected unethical conduct of employees, management, and other stakeholders in a confidential manner without any fear of harassment, intimidation or reprisal.

WHAT SHOULD A REPORT INCLUDE?

Whistle-blowers are encouraged to provide detailed information that will facilitate efficient and effective investigation. Such information should include, but are not limited to the following:

  1. Nature of the incident
  2. People involved
  3. Date of incident
  4. Place of occurrence
  5. How the incident occurred and
  6. Any other useful information.

However, potential whistle-blowers are not discouraged from blowing the whistle if they do not have all the required information highlighted above.
